Why botox…? We not only communicate verbally, but also use facial expressions: we smile and laugh, but also frown or become angry and surprised. These emotions visibly arise from contractions of the facial muscles. Over the years, some of these such as the laughter lines (crow’s feet) develop into wrinkles around the eyes as a mark of the ravages of time. Indeed, we generally grow one horizontal forehead line per decade of life, which might subsequently and inevitably reveal our age. Others, such as frown (glabellar) lines reflect negative emotions including anger. The use of botulinum-toxin smooths out these furrows which develop over time and give people a younger, more pleasing and more attractive look.

 

How does it work…? There are several sub-types of botulinum-toxin with slightly different characteristics: type-A is widely used in the daily medical routine. By inhibiting the release of signaling molecules (most notably acetyl-choline), it blocks the transmission of nerve impulses. As target organs respond differently to a nerve stimulus, the impact of interfering with neuro-transmission also varies depending on the areas affected. Therefore, botulinum-toxin may relax muscles, decrease the secretion of sweat glands, dilate blood vessels and increase blood flow, reduce pain (nociceptive nerve-block), etc. These effects last about 2-4 months after which time acetyl-choline is slowly released again, and the target organs (muscles, sweat glands, etc) are gradually reactivated.

 

What does it do…? The name comes from the Latin “botulus” meaning “sausage”, as the germ (Clostridium botulinum) that produces the toxin can cause severe wound infections or even a rare but potentially fatal food poisoning called botulism by growing in spoiled meat products. When contracted, the disease is primarily characterized by generalized muscle weakness (paresis) or even complete loss of motor function (paralysis) that gradually spreads down the body from the head to the legs, affecting the face and neck (blurred or double vision, drooping eye-lids, difficulty speaking or swallowing), chest (impaired breathing or even respiratory failure), and extremities (inability to move or walk, etc). However, the amount of botulinum-toxin (type-A) used in clinical settings is minimal and generally well tolerated: it is now routinely administered in conditions associated with spasms or painful and disfiguring muscle contractions, as well as extreme muscular activity such as cerebral palsy, cervical dystonia, blepharo-spasm, strabismus, and even over-active bladder or vaso-spastic disorders, eg. Raynaud phenomenon. By dilating spastic blood vessels and reducing nerve-compression caused by muscle contraction in the head, botulinum-toxin might also alleviate chronic migraine. Because it can reduce sweat-production of the armpits (axillae), palms of the hand, and the soles of the feet, it is even used to treat excessive sweating (hyper-hidrosis) which can be debilitating and socially isolating.

 

How is it used in facial esthetics…? The botulinum-toxin also temporarily weakens the muscles responsible for developing dynamic (hyper-functional or animation) facial expression lines, ie. wrinkles in motion. On the other hand, static facial lines, eg. those resulting from collagen or fat loss, gravity, sun damage, etc (wrinkles at rest) will not usually respond to treatment with botulinum-toxin as these are unrelated to muscle activity. In cosmetic medicine, the product is licensed for the temporary improvement of moderate-to-severe vertical lines between the eye-brows (glabellar lines) and is injected into the skin to reduce these. However, it can be applied in other areas as well (off-label), and the scope of botox in cosmetic use is increasing every year. After treatment, improvement should be seen within 2-3 days, however the full effect can take up to 30 days to develop and usually lasts between 4-6 months. For best practical purposes, botox injections are considered lunch-time procedures, where our clients can return to their normal daily routine immediately after the treatment. However, it is advisable for patients to avoid certain activities such as vigorous exercise, lying down for around 4 hours after the procedure, taking a hot shower as well as sauna or bathtub, as these may lead to blood congestion and bruising in injected areas, and wash out the product before it has a chance to interact with the targeted muscles.

As we grow, our body gradually loses crucial connective tissue components such as collagen, elastin, and hyaluronic-acid. Around the early 40s our facial fat-pads also start to decrease. In addition to these, gravity itself over the years causes the structures of our skin to sag, creating deep folds, droopy eye-lids, down-turned mouth and fleshy jowls, all contributing to that aged look we so clearly wish to avoid.

The PRP is then injected back into the skin to encourage new collagen formation and improve blood supply, both of which are important factors in anti-aging therapies. PRP also contains high levels of tissue growth and regeneration factors which are vital to skin rejuvenation and repair: skin texture and tone will improve as new cells grow. Early visible results may take 6-8 weeks to develop, however this revitalization continues over time: the final outcomes and benefits show within 6 months, and last between ½-2 years, but the best results are generally attained through 2-4 consecutive treatments.
